I don't know why this went wrong, any thoughts appreciated. Here's a pic of both sides the negs: http://www.londondailyphoto.com/images/IMG_2349.JPG
I normally process TMax (Tmax400 in this case) in T-Max Dev but as I had some D-76 left at the end of it's life, I used that. To be exact, I had about 800ml of D76 mixed 6 weeks ago, used once 5 minutes before to process a roll of Fuji Acros (perfect) I poured the 500ml from the Acros dev back into the container and used 500ml of that in the tank. 20.5oC, 7 min 10 s, Ilford rapid fixer - (end of use, too, but I didn't think it was exhausted).
Unfortunately, I always take care with times and chemicals so I've had no experience of failure, and I don't know what's happened. Any ideas?

Difficult to say from the picture in your kitchen sink! The one on the left looks un fixed, not even started to clear. The one on the right looks about right on the developer but still not completely fixed, if this is the Fuji I would say the fix is exhausted. Have you tried to refix both films with fresh solution, wont do any harm and you might still get some useable negs.

Thanks for the thoughts, the left and right are the same film (and it's T-Max 400 - all that about the Fuji Acros was the previous roll I had just done), just on both sides. And it was sitting in fix as I was typing but you are right - I've now gone for fresh stuff.

If it was all the same film then some of it must have been on the spiral incorrectly such that two layers of film were in intimate contact. This would stop solution getting to the emulsion and cause some of it to be un-developed and un-fixed. This would be confirmed when you refix, if that area becomes entirely clear.

No, you misunderstood what I meant about the pic. That's what the same film looked like from either side. Fixing again has cleared it slightly but it is still a pile of poo.
There is a note on the TMax datasheet "this will exhaust fixer more quickly than any other film" but, as I said, the Acros cleared perfectly. urgh.
